How to Get Help

Follow these easy steps to get mental and/or medical health support from the Teen Health Center for your child:

Mental Health Services

To arrange for your child to see a mental health professional:

  1. Complete the appropriate forms for your child: You can get these forms by walking into any clinic location [link to location page] or download them here. Completed forms can be submitted in person to one of our clinics or via mail to P.O. Box 925, Galveston TX 77553-0925 or faxed to 409-765-5026.
  2. Schedule an intake appointment: Once the paperwork for your child has been received, you will be contacted to schedule an intake appointment. If you bring your completed forms directly into a clinic, you can schedule this appointment in person.
  3. Schedule a follow-up appointment: Following intake, your child will be scheduled for an appointment with a doctor, therapist, or specialist for testing.

If the Teen Health Center determines that your child would be better served by another agency, the appropriate referrals will be made to ensure his/her needs are being met.

Medical Health Services

To arrange for a medical appointment for your child call one of the five medical clinics.  A signed consent form is required each year.  You may download the form here (link to form) and bring the completed form to the appointment.  If the appointment is for a sports physical, the patient should bring the physical form and $5.00 for the visit.
